Why Your GLP-1 May Have Stalled

If you're on a GLP-1 medication (Ozempic, Wegovy, Mounjaro, Zepbound) and things have plateaued, there's usually a real, physical reason underneath it.
I hear some version of this conversation almost every week in my practice. Someone started their medication with real hope. The first weeks or months worked exactly as promised, appetite quieted down, the scale moved, energy came back. Then, somewhere along the way, things flattened out. Maybe the weight loss slowed to a crawl. Maybe it stopped completely. Maybe the fatigue crept back in, or the number on the scale started climbing again even though they're eating less than ever.
The frustrating part is that most people are told the same thing when this happens: eat less, move more, be patient. But in my work as a functional nutritionist, I don't see a GLP-1 stall as a discipline issue. I see it as a signal. The medication is doing its job, quieting hunger signals and slowing digestion, but something underneath is preventing the rest of your metabolism from following along. My job is to figure out what that something is.

Here's what I look at with clients.
Insulin resistance that runs deeper than expected
For a lot of people, years of blood sugar swings, high triglycerides, or a fatty liver have been building quietly in the background, often without a diagnosis attached. Standard bloodwork can look "fine" for years while insulin resistance is developing underneath it, because a normal glucose reading doesn't tell you how hard your pancreas is working to keep it there.
A GLP-1 can blunt appetite beautifully and still not be enough to override significant insulin resistance on its own. When your cells have become resistant to insulin's signal, your body tends to store fuel as fat more readily and release it more reluctantly, no matter how little you're eating. This is one of the first places I look with a new client. Fasting insulin, HbA1c, triglycerides, and liver enzymes tell me a lot about how efficiently your body is actually using the fuel you give it, and they often explain a slow response long before anyone thinks to ask.
Inflammation that's working against you
Chronic, low-grade inflammation makes the body hold onto fat as a protective response, whether it's coming from gut issues, poor sleep, chronic stress, hidden infections, or even something like periodontal disease. Inflammation interferes with the same signaling pathways that insulin and appetite hormones rely on, so even a medication designed to quiet hunger can be working against a body that's in a constant, low-level state of alert.
The tricky part is that this kind of inflammation rarely comes with obvious symptoms. Someone can feel generally fine, aside from the puffiness, joint stiffness, or brain fog they've written off as normal, while their labs tell a very different story. It's rarely on anyone's radar until we go looking for it, and markers like hs-CRP, ferritin, and homocysteine can be incredibly revealing once we do.
Muscle loss and a metabolism that's quietly slowing down
When appetite drops sharply, it's easy to eat too little protein without realizing it, especially when food just doesn't sound appealing the way it used to. Muscle is metabolically expensive tissue, and it's part of what keeps your resting metabolic rate humming even when you're doing nothing at all. Lose it, and your body needs less fuel than it used to, which can look and feel exactly like a plateau, even though the scale might still be inching down.
This is especially common in women over 40, where muscle mass is already declining with age, and in anyone who's spent years cycling through diets, since each round of rapid weight loss tends to take a little more muscle with it than the last. The research on rapid weight loss from GLP-1s backs this up: a meaningful portion of what comes off can be lean mass, not just fat, if protein intake and resistance training aren't part of the picture. That's often the missing piece, not the medication itself.
Cortisol running high
I'll say it plainly: this is one of the biggest hidden reasons I see people stall. Chronically elevated cortisol drives insulin resistance, encourages fat storage around the midsection, disrupts sleep, and can dampen thyroid function. It also tends to increase cravings for the exact foods that work against your goals, which can feel like a willpower failure when it's really a hormonal one.
Cortisol doesn't need a single big crisis to stay elevated. It responds just as strongly to years of poor sleep, an overloaded schedule, or a nervous system that's simply never had the chance to fully rest. If your body has been in fight-or-flight for months or years, no medication can fully out-run that, because appetite suppression doesn't undo the stress response driving the fat storage in the first place. This is one of the reasons I ask clients about sleep and stress just as closely as I ask about food.
Thyroid function that looks "normal" on paper
A standard TSH can sit comfortably in range while Free T3 is low or Reverse T3 is elevated, meaning your body isn't converting thyroid hormone into the active form it needs. This conversion step matters enormously, and it's exactly what gets missed when a doctor only runs a TSH and calls it a complete thyroid picture.
Your metabolism can be running on far less energy than it should, even with "normal" labs, which is often why someone feels cold, tired, foggy, and stuck on the scale all at once, but keeps being told everything looks fine. A full thyroid panel, not just TSH, is often the difference between finding this and missing it entirely.
Shifting hormones, especially in perimenopause
So many women start a GLP-1 right in the middle of the perimenopause transition, when estrogen, progesterone, and testosterone are all in flux and muscle mass is naturally declining. Estrogen in particular plays a real role in insulin sensitivity and where the body prefers to store fat, so as it fluctuates and eventually declines, fat tends to shift toward the midsection even without any change in eating habits.
Layer a calorie deficit on top of that hormonal shift, and weight loss can move at a very different pace than it did in your 30s. This is one of the most common frustrations I hear: doing everything the same way that used to work, only to find the body responding completely differently now. It's not in your head, and it's not a sign that you're doing something wrong. It's a different hormonal environment that calls for a different approach.
A gut that isn't functioning well
Your gut influences inflammation, insulin sensitivity, nutrient absorption, and even your body's own natural GLP-1 secretion. This last point surprises a lot of people: your gut already makes its own GLP-1 in response to food, and an unhealthy gut environment can blunt that natural signaling, on top of everything the medication is trying to do.
Dysbiosis, SIBO, low levels of beneficial bacteria like Akkermansia, or ongoing constipation can all quietly work against the progress you're trying to make, often while showing up as symptoms that seem completely unrelated to weight, like bloating, irregular digestion, or skin issues. I see this constantly with clients who assumed their digestion was "normal" simply because it was familiar.
Not eating enough protein
This one is close to my heart because it's so common and so fixable. Appetite suppression is powerful, and it's easy to end up living on coffee, yogurt, crackers, and half a sandwich without ever noticing how little protein is actually in a day like that.
GLP-1s are meant to reduce hunger, not reduce nutrition, but that distinction gets lost when food simply stops sounding appealing. When protein intake drops too low, muscle loss and fatigue follow closely behind, and the scale stalls right along with them, because your body has less lean tissue to burn through calories and less raw material to repair and rebuild with. This is often the single biggest, and easiest, fix I make with clients.
Micronutrient gaps
Magnesium, iron, vitamin D, zinc, selenium, and B vitamins are often the first things to run low when intake drops overall, and each one plays a role in how efficiently your metabolism actually runs. Magnesium alone is involved in hundreds of processes in the body, including insulin signaling and energy production, so a quiet deficiency there can show up as fatigue, cravings, or a stalled scale with no obvious cause.
Low iron can leave you exhausted and short of breath during exercise, effect energy levels overall as well as effect hair growth.. Low vitamin D is linked to poorer insulin sensitivity, effects on bone health as well as your immune system to name a few areas needing Vitamin D at a optimal level. Low B vitamins can affect energy at the cellular level. Many of these are dismissed on a blood panel because our ranges are too broad, or not even checked at all. Which is why I run them specifically with clients who feel like something is off.
The dose or the medication itself
Sometimes it really is this simple: some people respond better to a dual agonist like tirzepatide than to semaglutide, and others simply need more time at a therapeutic dose before the full effect kicks in. Titration schedules exist for a reason, and the dose you're on at month one may not be the dose your body needs by month four.
This part of the conversation always belongs with your prescribing clinician, not with me, but it's worth naming as one possible piece of the puzzle so it doesn't get overlooked. If everything else checks out and the plateau persists, that conversation is worth having alongside the deeper root-cause work, not instead of it.
What I actually do about it
When someone comes to me saying their GLP-1 "just isn't working anymore," I don't start by questioning their effort. I start by running functional labs: fasting insulin, a full thyroid panel, inflammatory markers, iron & ferritin panel, Vitamin D, magnesium, and often a GI-MAP if digestion is part of the picture. From there, we build a plan that supports what the medication can't do on its own: rebuilding protein intake, calming inflammation, supporting the thyroid and adrenals, and protecting the muscle you've worked hard to keep.
GLP-1 medications are a genuinely powerful tool. But they were never designed to correct every root cause of weight loss resistance on their own. When insulin resistance, inflammation, thyroid dysfunction, shifting hormones, nutrient gaps, or an overworked stress response are part of your story, addressing those pieces directly is often what determines not just how well the medication works now, but how well you hold onto your results once you eventually come off it.
